Coordinate and manage care for Aetna members by working closely with healthcare providers, case managers, and other team members to ensure the best possible care is provided.
Conduct initial assessments and ongoing evaluations of members' healthcare needs to develop personalized care plans.
Communicate with members and their families to provide education and support regarding their healthcare options and resources.
Collaborate with healthcare providers to identify and address any barriers to care and develop solutions to improve the overall health and well-being of members.
Utilize electronic health records and other systems to document and track member progress and ensure accurate and timely updates are provided to all involved parties.
Monitor and review utilization of healthcare services to ensure appropriate and efficient use of resources.
Act as a liaison between members and healthcare providers to advocate for their needs, coordinate appointments and services, and address any concerns or issues that may arise.
Stay up-to-date on industry changes and advancements in healthcare practices to provide the highest level of care for members.
Collaborate with other Care Management Associates and team members to continuously improve processes and enhance the quality of care provided to members.
